School Nurse



Welcome to the Riverbank Charter School's Health Office webpage. 

I am looking forward to a happy and productive relationship with you and your child over the course of your child's time at Riverbank Charter School of Excellence. A very important part of the foundation of this relationship is insuring that your child is in the best of health before beginning school. Healthy children are ready and able to learn! Therefore, Riverbank Charter School of Excellence and the State of New Jersey require that you provide documentation of a physical exam and a record of immunizations prior to your child entering school. A link to required immunizations for New Jersey and a form for the physical exam can be found on this webpage.

While your child is here at Riverbank Charter School of Excellence we hope to contribute to maintaining your child's good health by conducting blood pressure, height, weight, vision, and hearing screenings annually. As the nurse working within the school, I also provide emergency first aid and screen for communicable diseases as they arise. Of course all of the expected tummy aches and sniffles are cared for as well!

Please keep me informed of any changes in your child's health over the course of the year so that I can make sure we are meeting all of your child here at school.

Administering Medication

The administration of prescribed medication to a student during school hours will be permitted only when failure to take such medicine would jeopardize the health of the student, or the student would not be able to attend school if the medicine were not made available to him/her during school hours.  Medication shall include all medicines prescribed by a physician, both over the counter and prescription. 

Before any medication is administered to or by any pupil during school hours a written request from the parent/guardian and the physician is required.  The request form is available in the School Nurse's office which must include the following:
  • The purpose of the medication
  • The dosage of the medication
  • The time at which the medication shall be given
  • The length of time for which the medication is prescribed
  • The possible side effects of the medications

The written request for prescribed medication will be kept on file in the School Nurse's office. All medications shall be administered by the School Nurse, or the Parent/Guardian. Students may not carry any medication on their person for purpose of self administration.
